Common causes

Post-exertional malaise is the hallmark feature of ME/CFS and a defining feature of long COVID. It is also reported in fibromyalgia and in POTS and other forms of dysautonomia, where being upright is itself a form of exertion. It is often mistaken for deconditioning, but deconditioning tends to improve with graded activity while PEM predictably worsens with it, and it is not ordinary tiredness, which follows effort immediately and clears with rest. Flares that resemble a crash can also come from untreated sleep apnea, anemia, thyroid disease, low vitamin B12 or D, uncontrolled diabetes, depression, medication effects (beta blockers, sedating antihistamines) or an ongoing infection. Less commonly, delayed weakness after effort points toward myasthenia gravis, a metabolic or mitochondrial myopathy, multiple sclerosis, heart failure or adrenal insufficiency.

When to see a doctor

Seek urgent care for chest pain or pressure with effort, breathlessness at rest, fainting rather than lightheadedness, sudden one-sided weakness, slurred speech, or cola-colored urine with severe muscle pain after activity, which can indicate muscle breakdown. A fever above 38.5°C (101.3°F) lasting more than three days, or unexplained weight loss alongside crashes, needs prompt review. Book a routine appointment if crashes have recurred for six weeks or more, if your baseline keeps dropping despite rest, if you are giving up work or self-care to stay within your limits, or if new features appear such as persistent swollen glands or worsening memory.
